Current Licensees and Applicants - Register for Dashboard
Important Notice to all Licensees and Applicants
The Board of Medicine is moving to a more secure login system with the launching of this website. As such, if you currently have a license or an incomplete, pending application - you will need to "register" for the new dashboard and create a username and password that you will utilize moving forward. This will also take you to a licensee dashboard where you can access your license information, update your contact information, and print your license.
NOTE: MD/DOs issued Provisional Temporary Licenses will not be able to register until they receive their approved, full license after the Board meeting.
If you are a NEW applicant for licensure, DO NOT register here as you will be asked to set up your username and password during your initial application process.
To register, click on your license type below and enter your current credentials.
For current licensees, this will be your last name, last 4 digits of your social security number, and license number.
- MD/DO - only put in the license number - no preceding letters. (ex: "12345").
- MD/DO Administrative License - put in entire license including preceding "A" (ex: "A12345")
- Residents - put in entire license including preceding letters (ex: "RL12345").
- PA/GC - put in entire license including preceding letters (ex: "PAC1234").
- Naturopaths - put in your last name, license number, and password previously used with the Board of Integrative Health.
- If you do not remember your license number, you can look it up: Verification of Licensure.
For pending applicants, this will be your last name, last 4 digits of your social security number, and date of birth (mm/dd/yyyy). Note that this is only for applicants who did not create a username and password when submitting their initial application (i.e. applications submitted before July 10, 2023). If you created a username and password with your application, you are already registered and can utilize the Dashboard login.
After logging in with your current credentials, you will be prompted to create a username and password. You may chose any username, there are no requirements. Passwords must be at least 8 (eight) characters in length and include at least 1 (one):
- uppercase letter,
- lowercase letter,
- number, and
- one of the following special characters: @ ! $ % & * ( ) + - [ ] { } , / : ?
You will also be asked to provide a cell phone number. For security reasons, if you forget your password and need prompting, this notification will go to the cell phone number provided.
After successful registration, use the username and password to login to your Licensee Dashboard thereafter.
