Types of Licenses

Full License  

Full licenses are only granted/approved on the day of regularly scheduled Board meetings.

Administrative License  

Administrative licenses are issued for the purpose of practicing only in an administrative capacity and does not allow an individual to practice clinically.

Provisional Temporary License

A provisional temporary license may be issued to an applicant who has met all requirements for licensure and granted in the interim between regularly scheduled Board meetings to allow an individual to start practicing. A provisional temporary license is only valid until the time of the next regular meeting of the Board, at which time the Board will review to approve the license.  Once approved, the license automatically converts to either a full or administrative license.  Note that when this happens, the “PT” drops off and the license retains the 5 digit license number as their permanent license number (example: moving "PT12345" becomes “12345”).  Administrative licenses will convert to “A” followed by the 5 digits (example: “A12345”).

Locum Tenens Permit

Locum tenens permits are issued for a specific practice site and for a period not to exceed three (3) months.  A locum tenens license can be converted to a provisional temporary (PT) license by submitting an additional $200 fee.

Uniquely Qualified License

The Board has the discretion to issue this license to an application for physician licensure who does not meet all the technical eligibility requirements for licensure if the Board determines the applicant is uniquely qualified through training or experience or will make a unique or special contribution to the practice of medicine not readily available to the citizens of the state.  The Board will consider whether the applicant is national board certified, the nature and length of medical practice and postgraduate training/research, whether the applicant is licensed in good standing with no discipline in another jurisdiction, malpractice judgements or settlements, and whether the applicant has passed the USMLE or SPEX examinations.  Applicants wishing to obtain this licensure will need to petition the Board for approval of the license which will be reviewed at the Board's next regularly scheduled meeting. 

Special Emeritus Certification  

This certification is considered “honorary” and does not allow the individual to practice medicine.  There is no fee for this certification.  To obtain this certification, please email the Board.
